The Town of Oxford Massachusetts is seeking proposals from qualified individuals or firms to provide “Title V Inspection Services” in support of the Board of Health (Board) responsibilities under the State Sanitary Code, 310 CMR 15.00.
The required services include: witnessing soil evaluations and percolation tests and documenting results; reviewing construction drawings for on-site sewage disposal systems and documenting whether the proposed plans comply with the requirements of Title V; reviewing plans for the installation of private wells and documenting whether the proposed plan complies with Title V and the Board’s well installation regulations; making recommendations to the Board on the need for local upgrades or variances; attending Board meetings to assist the Board in the consideration of upgrades and variances; inspecting construction of on-site sewage disposal; reviewing and documenting whether “as-built” plans show compliance with Title V; inspecting sites of possible Title V violations; and all other related services.
The minimum requirements for consideration are: the individual(s) performing the services shall be a certified soil evaluator who is thus qualified to witness soil evaluations and percolation tests; the individual(s) performing the services shall be a professional engineer who is thus qualified to review plans for pumped and pressure dosing systems; the individual(s) performing the services shall be someone who does not also do work in Town; the individual(s) performing the services must have daily availability to the Board of Health clerk; the individual(s) performing the services shall be able to provide timely inspections of installations and complaints; and the fee structure is a per unit cost (rather than hourly plus expenses).
Proposals shall include: the qualifications of the individual or individuals who will be performing the required services; a fee structure for the services; a list of references; and a statement of availability to perform the services.
Late submissions will not be accepted, although during the evaluation process, the Town reserves the right to request additional information or clarification from proposers, or to allow correction of errors or omissions.
The Town reserves the right to accept or reject any or all proposals and to make an award deemed to be in the best interest of the Town of Oxford.
If any addenda are issued for the Request for Proposals, a good faith effort will be made to deliver a copy to each of those potential proposers who, according to the Town records, have previously received a copy of this RFP. However, it shall be the sole responsibility of those offering proposals to contact the Town Manager’s Office prior to submitting their proposal to determine whether any addenda or clarifications have been issued.
Proposals are to be submitted in a sealed envelope clearly labeled and will be accepted until 4:30 pm on Tuesday May 2, 2006 in the Town Manager’s Office, 325 Main Street, Oxford, MA 01540.
