Records Request Form

Use this form to request copies of official reports. All requests are to be submitted to the Administrative Assistant Debrah Light. This form is to request a copy of an Arrest Report, or a Police Report from the Oxford Police Department.

Vacation Notification

Use this form to ensure the Oxford Police has important contact information, just in case there is an emergency at your residence while you are away.

No Trespass Form

This form is used to formally notify someone that they may not enter or remain upon your property. Once a person receives notification of a No Trespass, a violatior is subject to arrest and a criminal charge of Trespass (Mass General Law Chapter 266, section 120). Only the rightful owner of a property may issue a No Trespass.

Accident Form

You should file this form if you were the operator of a vehicle involved in a crash where the damage to any one vehicle or property was over $1000, or if there was an injury to any person, even if a police officer was on scene. You do not need to file this form if the crash occurred on a private road, driveway, private parking lot or other private way.

Reassurance Program

This program was designed for Oxford's Seniors or citizens with special needs. People enrolled in this program call and check in with the Police Dispatcher every morning. In the event that a person fails to call and check in, Dispatch will first call the person's house and if no one answers, a Cruiser is sent to check on the that person's well being.

911 Disability Indicator FormThe filing of this document with your 9-1-1 Municipal Coordinator will alert public safety officials that an individual residing at your address communicates over the phone by a TTY and/or has a disability that may hinder evacuation or transport.
